I have a few questions about the Hazard / Risk Evaluation Procedure on page 77 and 79 of the 2009 edition - 70e.

1 - What does the probability factor mean ?
Probability of an individual doing the task ?
Probability of making a mistake ?
Probability of things going wrong ?

2 - What does the CI in the Class column mean ?

3 - Is the Severity number 1-4 based on a flash analysis or shock hazard , or both ?

I was asked to explain this and was at a loss for these items. Any help will prevent this from happening again
